Travel Sheet Preferences

The Travel Sheet preferences tab is where you look to customize your Travel Sheet/Encounter Form/Superbill. It is from this preferences tab that you can add/remove specific items from the form, such as the patient's memo and patient's next appointment. You can also configure the software to use your own customized form/image to be viewed when you print the form from the Forms menu.

Refer to the following screenshot and table as a reference for determining how each checkbox affects the way the Travel Sheet looks when you print one.

Patient Number - Uncheck to remove the patient's identification number from the form.   Patient Name -  Uncheck to remove the patient's name from the form. Patient Address - Uncheck to remove the patient's address from the form. Patient Phone - Uncheck to remove the patient's telephone number from the form. Patient File Date - Uncheck to remove the patient's file date from the form. Patient Gender - Uncheck to remove the patient's gender from the form. Patient SSN - Uncheck to remove the patient's social security number from the form. Patient DOB - Uncheck to remove the patient's date of birth from the form. Patient Provider - Uncheck to remove the name of the patient's primary provider from the form. Patient Guarantor - Uncheck to remove the name of the patient's from the form. Patient Referred By - Uncheck to remove the name of the patient's referrer from the form. Patient Employer - Uncheck to remove the name of the patient's employer from the form. Patient Balance Due - Uncheck to remove the due balance on the form. Patient Next Appointment - Uncheck to remove patient's next scheduled appointment from the form. Patient Memo - Uncheck to remove the patient's memo (from the patient information screen) from the form. Load Custom Image - Click to browse and open customized image to print onto the bottom of the form (.bmp format only). Load Custom Form - Click to browse and open a customized list of diagnosis to print onto the form (.rtf format only). No color/shading - Check to remove any extra shading/color from the form. (will help to conserve ink/printer toner).
